Medical Refunds

If you wish to apply for a medical refund for an event, please carefully read the following form, fill in the details and send as directed (with attached documents).

Riders need to fill out the form and attached it with their medical certificate and send to bmxnswentries@gmail.com within 1 week of the event.

Fill in the form
Save it as your name
Take a photo of your medical certificate
Email both the form and your photo to bmxnswentries@gmail.com
